Below you will find new patient paperwork, including a comprehensive health history, personal information, the privacy policy, and a consent for treatment.  Fill out this form and bring with you to your first visit.  If you have any questions, we can discuss them during your visit.  If you plan to use insurance for your acupuncture treatment, please bring your insurance card with you.

I need to get a comprehensive picture of your health status, so please allow one-and-a -half hours for your first visit.  The follow up treatments will last about an hour.
